If you do not want your health records to be shared for research and planning purposes there is now an easy way for you to opt out.

You can find out more on www.nhs.uk/your-nhs-data-matters

You will need your NHS no. to be able to do this, and your mobile phone number should be up-to-date on your medical records to enable your records to be matched with your request.

Alternatively you can ring the telephone support service on 0300 303 5678.
